Care-A-Van Transportation Services
Description: Care-A-Van offers door-to-door transportation and errand service to persons with disabilities who are unable to ride the City bus and to anyone age 60 or older. Care-A-Van provides service Monday through Saturday, 7 a.m. until 11:30 p.m. and on Sunday, 9 a.m. until 6 p.m. Please call (907) 463-6194 to reserve a ride. The reservations office is open Monday through Friday, 8 a.m. to 4:30 p.m. and Saturdays, Sundays, and holidays 2 p.m. to 4 p.m. Care-A-Van is provided by Catholic Community Service.

Description: A program of Catholic Community Service, the Douglas Senior Center offers the following services: congregate meals at the Senior Center; health promotion activities (blood pressure checks, flu shots, and health information); social and cultural activities; and a Senior Information/Caregiver Resource Center. The Douglas Senior Center is open for service 11 a.m. to 1 p.m. Tuesdays and Thursdays.

Description: The Foundation for End of Life Care is a Juneau-based organization with a mission to support end of life services including hospice, bereavement care, and community education. This support can be in the form of financial grants to organizations and agencies that provide hospice care, end of life care, and/or bereavement services to patients and families. This includes community education about end of life issues through special publications and presentations, speaking engagements, and special projects. The agency’s primary goals are to heighten community awareness toward improving the quality of the end of life and to responsibly and wisely invest and manage the Foundation for End of Life Care Fund in order to ensure there will always be monies to support the Foundation’s mission.

Description: Hospice and Home Care of Juneau (HHCJ) is a program of Catholic Community Service. When referred by a physician, patients can receive a variety of health care services at home through HHCJ. Professional services such as nursing, physical therapy, occupational therapy, speech therapy, and social work are provided to qualified home health patients in conjunction with personal care, when needed, delivered by Certified Nurse Aides. Additionally, HHCJ offers hospice care at home to patients diagnosed with a terminal illness. HHCJ provides the community with several options for addressing bereavement issues related to a loss through death other than our in-home, physician prescribed services. Adult bereavement support groups offered in the fall and spring meet weekly for eight weeks each. Also, at least one children’s and family grief support group is offered annually depending on need.

Description: Wildflower Court is a nonprofit, independent, and self-financed long-term care nursing home. Situated in Juneau, Wildflower Court offers services to many Southeast Alaska communities. Wildflower Court is able to accommodate 49 residents allowing them to remain in Southeast Alaska and be closer to their families and loved ones. The 4-year-old facility is located above Bartlett Regional Hospital in the Salmon Creek area.
